Health insurance

INFORMATION ON HEALTH INSURANCE IN THE TERRITORY OF THE REPUBLIC OF POLAND FOR FOREIGN STUDENTS AND PhD STUDENTS

EU OR EFTA CITIZENS

The students and PhD students who are citizens of EU or EFTA member states receive medical care in the territory of the Republic of Poland free of charge, pursuant to the provisions on the coordination of social security systems, as far as they are covered by health insurance in the common social security system of an EU or EFTA member state. The documents that make their holder eligible for healthcare free of charge on the same terms as Polish citizens are: a valid passport, a valid student or PhD student ID, and a valid European Health Insurance Card.

NON-EU OR NON-EFTA CITIZENS OF POLISH ORIGIN

The students and PhD students who are not the citizens of EU member states but are recognised as Poles in the meaning of the provisions on repatriation or hold a valid Polish Charter, if they intend to sign up for health insurance via the University of Wroclaw are obliged to conclude an agreement on voluntary health insurance coverage in a department of the National Healthcare Fund (NFZ) competent for their residential address and to fill in and submit in the Dean’s Office/educational unit appropriate for their field of studies: a health insurance application form and a student’s/PhD student’s declaration on a lack of health insurance coverage from other sources.

A foreign student/PhD student of Polish origin in the meaning of the provisions on repatriation, in order to conclude a voluntary health insurance coverage agreement and to register for health insurance via the University of Wroclaw should:

1. collect from the appropriate Dean’s Office a certificate on their student/PhD status;

2. submit a voluntary health insurance coverage application in a department of the National Health Fund competent for their residential address and conclude a voluntary health insurance coverage agreement with the National Health Fund. The department of the National Health Fund competent for Wroclaw residents is located at ul. Joannitów 6 in Wroclaw. The voluntary health insurance coverage application form is available there at the site.

While submitting the documents the student/PhD student should carry with them:

– an ID (passport);

– certificate on a student/PhD student status;

– proof of residence in Wroclaw;

– PESEL certificate (if PESEL has been granted).

3. Within 3 days from the day the agreement with the NFZ was signed fill out in the appropriate Dean’s Office a health insurance registration form ZUS ZZA and a student’s/PhD student’s declaration on a lack of health insurance coverage from other sources.

The appropriate Dean/educational unit submits to the Student Affairs Department the aforementioned documents submitted by the student/PhD student.

If there is no way to verify insurance with a healthcare provider via e-WUŚ system, a copy of the agreement along with a proof of contribution payment from the last month consitute the proof of insurance. Those documents should be presented along with an ID. The proof of contributions payment made by the University of Wroclaw for the student/PhD student is issued by the Student Affairs Department.

The students and PhD students who are not EU citizens but are of Polish origin in the meaning of the provisions on repatriation and registered for health insurance by the University of Wroclaw can benefit from healthcare services on the same terms as Polish citizens.

NON-EU AND NON-EFTA CITIZENS

The students and PhD students who are not EU or EFTA citizens and have not been recognised as Poles in the meaning of the provisions on repatriation can conclude with the National Health Fund an agreement on voluntary health insurance coverage and individually pay their monthly health insurance contribution.

A student/PhD student who is not an EU or EFTA citizen and has not been recognised as Pole in the meaning of the provisions on reptriation as well as does not hold a valid Polish Charter, in order to conclude an agreement on voluntary health insurance coverage and gain access to health services in Poland should:

1. collect from the appropriate Dean’s Office a certificate on student/PhD student status;

2. submit a voluntary health insurance coverage application in a department of the National Health Fund competent for their residential address and conclude a voluntary health insurance coverage agreement with the National Health Fund. The department of

the National Health Fund competent for Wroclaw residents is located at ul. Joannitów 6 in Wroclaw. The voluntary health insurance coverage application form is available there at the site.

While submitting the documents the student/PhD student should carry with them:

– an ID (passport);

– certificate on a student/PhD student status;

– proof of residence in Wroclaw;

– PESEL certificate (if PESEL has been granted).

3. After the agreement on voluntary health insurance coverage has been concluded with the NFZ, the student/PhD student should submit to a Social Security Office (ZUS) competent for their residential address a ZUS ZZA form within 7 days from the date the agreement was concluded. For each month of health insurance coverage the insured person is obliged to submit a ZUS DRA declaration and pay contributions to the ZUS account.

If there is no way to verify insurance with a healthcare provider via e-WUŚ system, a copy of the agreement along with a proof of contribution payment from the last month consitute the proof of insurance. Those documents need to be presented with an ID.

The students and PhD students who are not EU or EFTA citizens and have not been recognised as persons of Polish origin in the meaning of the provisions on repatriation and do not hold the Polish Charter, can benefit from healthcare services on the same terms as Polish citizens after the procedure above has been completed.
